The Sweetgrass Health Centre is one of the newly created Indigenous Interprofessional Primary Care Teams being developed across the province and is operated under the governance and guidance of the Shkagamik-Kwe Health Centre.
How
We Came To Be
The AHACs were created under the Aboriginal Health and Wellness Strategy (AHWS) with culture at the core of the development.
AHWS conducted and extensive provincial consultation process which led to the creation of the AHWS Wellness model with key culture-based indicators. Since the Genesis of the AHACs, the sector collectively developed and continue to evolve a culturally specific model of care which the sector as a whole has adopted into individual service models.
Where
We Are Going
The main purpose of Sweetgrass Health Centre is to create and deliver services that will prevent ill health, treat illness and provide ongoing support and aftercare. Services will be offered in a culturally safe way that welcomes, accepts and represents all Aboriginal peoples, utilizing an approach that combines Western and Traditional practices and reflects First Nation, Métis and Inuit culture, values and tradition. In addition, Sweetgrass Health Centre will maximize its land base usage to honour our inherent relationship to the lands for the traditional healing approaches.
Who we Serve
Sweetgrass Health Centre (SGHC) is an Aboriginal Health Access Centre (AHAC) dedicated to providing equal access to quality health care to First Nation, Métis and Inuit individuals and their families who live in the Township of Cochrane, or who reside onTaykwa Tagamou Nation, Constance Lake First Nation or Wahgoshig First Nation.
We are a culturally based wholistic health centre dedicated to leading balanced and healthy lifestyles. Programs and services support traditional values encompassing the connectedness of emotional, spiritual, physical and mental well-being.
